What to do when the passing of a loved one has occurred or is imminent?

If a passing has occurred or is expected, please contact us immediately at 516.418.7000 or by filling out the form below. If your loved one has passed outside of a hospital or hospice care, you must notify the authorities so that the death can be legally pronounced. If paramedics are not present, please call 911. The sooner you contact us, the more we can help.
